Responsibilities

• Conduct thorough Physical Therapy assessments during home visits.
• Provide direct patient care to evaluate medical conditions and rehabilitation potential.
• Utilize EMR documentation for accurate record-keeping.
• Maintain an unencumbered Physical Therapy license for New York.

Requirements

• Graduate from an accredited Physical Therapy (PT) program.
• Possess a valid Physical Therapy (PT) license in New York.
• Demonstrate proficiency in EMR documentation.
• Hold current CPR certification.
