Paradigm Rehab is seeking a dedicated Rehabilitation Therapist to join our compassionate team. As a Physical Therapist, you will conduct home visit assessments, provide direct patient care, and collaborate with a supportive community-focused team.
Responsibilities
- Conduct Physical Therapy assessments and provide direct patient care.
- Document patient progress accurately using EMR systems.
- Collaborate with patients and their families to develop rehabilitation plans.
- Utilize your expertise to assess medical conditions and functional limitations.
Requirements
- Completion of an accredited Physical Therapy program.
- Licensed Physical Therapist in New York state.
- Proficiency in CPR certification.
- Experience with EMR documentation (Hello Note experience preferred).
