Responsibilities

• Perform Physical Therapy assessments in patients' homes to evaluate medical conditions and rehabilitation potential.
• Provide direct patient care, focusing on functional capabilities and limitations.
• Document patient progress accurately and efficiently using electronic medical records (EMR) systems.
• Maintain CPR Certification and adhere to professional standards of care.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficiency in EMR documentation preferred.
• Compassionate and dedicated to patient care.
