Job Description

We are looking for a dedicated RN Care Coordinator to join our Ambulatory Care team at Denver Health. As an RN Care Coordinator, you will play a vital role in facilitating and coordinating care for a group of patients, ensuring quality outcomes and effective care delivery within established parameters.

Responsibilities

• Develop patient-centered goals and care plans.
• Collaborate with patients, families, and healthcare teams to implement and evaluate care plans.
• Coordinate care within clinic and across healthcare settings.
• Advocate for patient benefits and navigate potential barriers to care.
• Promote preventative care and health education.

Requirements

• Associate's Degree in Nursing and RN license in Colorado.
• Minimum 3 years of clinical experience in healthcare settings.
• Knowledge of case management principles and care coordination processes.
• Strong communication and problem-solving skills.
• Proficiency in electronic health records and Microsoft Office.
