SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join LifeBridge Health, a leading healthcare provider in Maryland. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges they may face. This role involves assisting with adjustment to institutional living, financial concerns, family relationships, counseling related to medical diagnoses and treatments, discharge planning, and end-of-life support.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and address common social, emotional, and psychological challenges.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's in Social Work (BSW) required, Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification required.
• Experience in a healthcare setting is a plus.
