SOCIAL WORKER

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments, addressing a variety of patient and family needs such as adjustment to medical conditions, financial concerns, family relationships, aging-related changes, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support the transition to post-care settings.
• Collaborate with multidisciplinary teams to address patient needs.
• Promote emotional well-being and enhance quality of life for patients.

Requirements

• Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification.
• Excellent communication and interpersonal skills.
