Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. Responsibilities
- Conduct psychosocial assessments and provide counseling to individuals and families.
- Assist with discharge planning and support patients in coping with medical diagnoses and treatments.

Requirements
- Bachelor's in Social Work (BSW) required; Master's degree and LCSW-C certification preferred.
- LGSW certification is a plus.
