Job Description
We are looking for a dedicated and compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, ensuring their emotional and social well-being during challenging times. This position offers a sign-on bonus potential of $15,000.
Responsibilities
- Offer counseling and support to patients and families facing various challenges.
- Assist in discharge planning and end-of-life counseling.
- Address financial concerns and family relationship issues.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Requirements
- Bachelor's degree in Social Work (BSW) required.
- Master's degree and LCSW-C certification preferred.
- LGSW certification is a plus.
- Strong communication and interpersonal skills.
