Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a full-time Social Worker, you will provide clinical social work services to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and support patients during transitions.
- Work collaboratively with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
