SOCIAL WORKER

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common patient/family challenges such as adjustment to institutional living and financial concerns.
• Assist patients/residents in understanding medical procedures and treatments.
• Offer counseling related to aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's in Social Work (BSW); Master's degree preferred.
• LCSW-C preferred.
• LGSW certification.
