Job Description

We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide critical clinical social work services and psychosocial assessments to patients and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Support patients and families with various challenges related to healthcare and emotional well-being.
• Assist with discharge planning and counseling on end-of-life care.

Requirements

• Bachelor's in Social Work (BSW) required.
• Master's degree in Social Work preferred.
• LCSW-C certification preferred.
• LGSW certification required.
