Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges such as adjustment to institutional living, family relationships, discharge planning, and counseling on end-of-life matters. Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments to address common problems.
- Assist patients with adjustment to institutional living and family concerns.
- Engage in discharge planning and counseling related to end-of-life issues.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C preferred.
- LGSW certification.
