Job Description

We are looking for a dedicated and compassionate Social Worker to join our team at LifeBridge Health. As a vital member of our healthcare facility, you will provide clinical social work services and psychosocial assessments to patients, families, and residents, addressing a wide range of common concerns and challenges. This Full-time position is located in Baltimore, MD at Levindale Hospital. The successful candidate will play a crucial role in supporting individuals through various life transitions, discharge planning, and counseling services.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Assist patients and families in coping with institutional living, financial issues, and family dynamics.
• Offer counseling and support related to medical diagnoses, aging, powerlessness, and end-of-life issues.
• Participate in discharge planning and collaborate with interdisciplinary teams.

Requirements

• Bachelor's degree in Social Work (BSW) required. Master's degree preferred.
• Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
• Compassionate and empathetic approach to patient care.
