Job Description
We are seeking a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a full-time Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. Your role will involve assisting individuals with adjustment to institutional living, financial concerns, family relationships, medical understanding, aging-related changes, discharge planning, and end-of-life counseling.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address patient/residents' and families' common social and emotional concerns.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with healthcare professionals to ensure holistic patient care.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
- Compassion, empathy, and strong communication skills.
