Job Description
We are currently seeking a compassionate and dedicated Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. The position involves addressing various issues such as adjustment to institutional living, financial concerns, family relationships, end-of-life care, discharge planning, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients/residents and families.
- Assist individuals in coping with challenges related to medical diagnoses, aging, and end-of-life care.
- Collaborate with healthcare teams to develop and implement effective care plans.
- Advocate for patients' needs and provide appropriate community resources.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong communication and interpersonal skills.
