Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. This full-time position based in Baltimore, MD, at Levindale Hospital involves providing essential clinical social work services to patients and families, assisting with various challenges and transitions.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Address issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life planning.
- Collaborate with healthcare teams to develop comprehensive care plans and discharge strategies.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred; LGSW certification required.
- Strong communication and interpersonal skills.
- Compassionate and empathetic approach to patient care.
