SOCIAL WORKER PRN

Job Description

We are currently seeking a Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common issues related to institutional living, financial concerns, family dynamics, medical understanding, aging, discharge planning, and end-of-life support.

Responsibilities

• Deliver clinical social work services and psychosocial assessments.
• Address adjustment to institutional living, financial concerns, family relationships, and more.
• Provide counseling and support relating to medical diagnoses, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree and relevant professional knowledge.
• 1-3 years of experience in a similar role.
• LBSW/LMSW/LGPC certification.
