Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our esteemed team at LIFEbridge Health in Baltimore, MD. As a key member of our Allied Health team, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common concerns such as adjustment to institutional living, financial challenges, family relationships, and end-of-life counseling. This position requires a solid educational background and 1-3 years of experience, along with licensure such as LBSW/LMSW/LGPC.

Key Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Assist patients and families with various emotional and practical challenges.
• Support patients in navigating complex medical diagnoses and procedures.
• Provide counseling on end-of-life issues and discharge planning.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• Licensure as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
• Empathy and a compassionate approach to patient care.
