SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common problems including adjustment to institutional living, financial concerns, and counseling related to death and dying. Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and address common problems in healthcare settings.
• Assist patients with adjustment issues, financial concerns, and discharge planning.
• Offer counseling related to aging, powerlessness, and end-of-life care.

Requirements

• Bachelor's degree or equivalent with 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of theory and practice in social work.
• Ability to provide compassionate care and support to patients and families.
