Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, Maryland. As part of our Allied Health department at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist with discharge planning and end-of-life support.
- Work collaboratively with interdisciplinary teams for patient care.
- Provide support and guidance to patients and families facing medical challenges and aging-related issues.
Requirements
- Bachelor's degree in Social Work (LBSW/LMSW/LGPC).
- 1-3 years of relevant experience in clinical social work.
- Strong knowledge of social work theory and practice.
- Ability to provide compassionate care and support to patients and families in challenging situations.
