SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. The selected candidate will provide clinical social work services to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, discharge planning, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist patients with understanding medical procedures and treatment plans.
• Collaborate with healthcare teams to develop discharge plans.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in a healthcare setting.
• Strong knowledge of social work theories and practices.
