Job Description
We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, financial concerns, family relationships, medical procedures, aging, and end-of-life issues.
Responsibilities
- Offer psychosocial support and counseling to patients and families.
- Conduct assessments and develop care plans based on individual needs.
- Collaborate with healthcare teams to ensure comprehensive patient care.
- Assist with discharge planning and provide support during difficult transitions.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Knowledge of social work theory and best practices.
