SOCIAL WORKER PRN

Job Description

We are seeking a compassionate and dedicated Social Worker PRN to join our healthcare team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges they may face during their medical treatment and institutional living.

Responsibilities

• Conduct psychosocial assessments to identify patient and family needs in different areas of concern.
• Provide counseling and support related to adjustment to institutional living, financial issues, family dynamics, medical procedures, aging, and end-of-life planning.
• Collaborate with healthcare professionals to develop comprehensive care plans and ensure patient well-being.
• Assist in discharge planning and liaise with community resources to facilitate post-discharge care.

Requirements

• Bachelor's degree in Social Work or related field.
• LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience.
• Strong interpersonal and communication skills.
