SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, families, and residents. Your responsibilities will include addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling services.
• Assist patients and families with discharge planning and decision-making.
• Collaborate with healthcare teams to ensure comprehensive care.
• Offer support and guidance to individuals facing medical challenges and aging-related issues.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
• Strong interpersonal and communication skills.
