Job Description

We are looking for a compassionate and skilled PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face.

Responsibilities

  • Offer psychosocial assessments and clinical social work services to support patients and families.
  • Address common problems such as adjustment to institutional living, financial concerns, and family relationships.
  • Provide counseling on medical conditions, aging, discharge planning, and end-of-life issues.

Requirements

  • Bachelor's degree in Social Work or related field.
  • 1-3 years of relevant experience.
  • Licensed as LBSW, LMSW, or LGPC.
