SOCIAL WORKER PRN

Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Provide counseling on issues related to aging, discharge planning, and bereavement.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Adhere to professional standards and ethics in social work practice.

Requirements

• Bachelor's degree in social work or related field.
• Valid LBSW, LMSW, or LGPC license.
• 1-3 years of relevant experience.
