Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life planning.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Participate in discharge planning and counseling for patients and families.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
