SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services to patients and families.
• Assist individuals in coping with medical diagnoses, discharge planning, aging-related issues, and end-of-life concerns.
• Collaborate with healthcare teams to address patient psychosocial needs and facilitate appropriate interventions.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a Licensed Social Worker (LBSW/LMSW/LGPC).
• Understanding of psychosocial assessment and counseling techniques.
• Compassionate attitude and strong communication skills.
